I hereby grant my full consent and authorization for the above-named minor to engage in activities offered by Ris3 Academy I certify I am the parent or legal guardian of the above-named child; that I have read and understand the foregoing “Participant Waiver and Release; and that, in consideration of Ris3 Academy allowing this child to participate in the activities or use of facilities, I join in the waiver and release without reservation and agree to release and waive any claim or right of recovery I might have arising out of any injury, death or damage this child may sustain as against the Ris3 Academy its officials, employees and agents. I agree photographs taken of the child during such activities may be used for promotional purposes.

As parent/legal guardian, I authorize a qualified physician to examine the above named student and in the event of an injury to administer emergency care and arrange for any consultation deemed necessary to insure proper care of any injury. Every effort will be made to contact the parent/guardian to explain the nature of the problem prior to any involved treatment. I understand that I assume full responsibility for payment of any services rendered.

Ris3 Athletic Academy has been making a positive impact in the lives of hundreds of kids for over the years. We offer low-cost basketball clinics, camps, leagues, and tournaments to kids of all ages and skill levels. We also provide mentoring, tutoring, and scholarships to help our players succeed on and off the court.
